Initial Comments:
An offsite follow-up survey conducted on October 4, 2021, found that Salus Health, Inc., had corrected the following deficiencies cited under 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of Medicarea recertification survey completed on August 3, 2021.

The deficiency was cited as a result of a Medicare recertification survey completed on August 3, 2021.
